Under direction of the team leader and/or charge nurse, collaborates with coworkers on a daily basis to help ensure uniform standards of re and is responsible for performing duties for assigned nursing unit.  Demonstrates competency in the care and/or service to our patients/customers. Performs and prioritizes assigned duties and procedures in accordance with established methods and techniques as defined by policy.  Demonstrates the ability to provide a therapeutic environment to the mental health population. Responsible for observing, encouraging, and documenting patient involvement and actions during the visit. Responsible for providing a safe environment, minimizing risks, utilization of de-escalation tactics, and due to the high visibility of the patient pupation is often the first to respond to a behavioral outburst or medical issue. Responsible for understanding the sid effects of psychotropic medications.  Responsible for performing clerical duties and maintaining customer records for assigned nursing unit.


    • Communication
      • Answers and responds to call lights and phones promptly.  
      • Exhibits proper customer service skills with all customers, families and co-workers.  Strives to exemplify the St. Joseph Difference in all aspects of work.
      • Observes and reports changes in patient status to appropriate nurse.
      • Promptly responds to patient needs in a courteous manner.
      • Notifies the physician, correctly enters physician and ancillary department consults into Meditech and consult log.
      • Reports to oncoming tech and charge nurse at beginning and end of shift.
  • Establishes effective working relationships by communicating clearly and effectively with customers, physicians, hospital departments and co-workers.
      • Communicates with the MHMR representative to help establish a safe environment and plan of care for the mental health patient population.
  1. Performs/Provides Patient Care as Delegated by the Nurse
      • Gives basic bedside nursing care (e.g., baths, linen change, oral care, back rubs, toileting and repositioning patients).
      • Assists patients with ambulation and transfer to/from wheelchair, stretcher and/or bed, using proper body mechanics. Transports patients to rooms, other departments and vehicles.
      • Reapplies and removes restraints as directed by the nurse.
      • Sets up room for new admission and assists with orientation of room, equipment and supplies.
      • Performs accuchecks including quality control checks.
      • Obtains and transports specimens to the lab.  Obtains blood from the lab when necessary.
      • Verifies diet list and trays for accuracy before passing out meal trays.  Feeds/assists patients with meals.
      • Obtains VS, daily patient weights and I&Os.
  • Encourages patient to use Incentive Spirometer.
      • Provides a safe environment for the mental health patient population.
      • Ensures all personal belongings are kept in a secure are and labeled
  1. Documentation
      • Documents patient care correctly in Meditech.
      • Assembles charts for new admissions. Places ID label on all pages of medical record. Obtains old charts from Medical Records. Copies charts for patient transfers.  Ensures chart has adequate supply of forms.
      • Enters and transcribes physician orders correctly in Meditech and Kardex, correcting attending MD and adding consulting MD as requested.
      • Performs patient discharge in Meditech, using appropriate disposition code.
      • Scans physician orders to the pharmacy.
      • Enters work orders into Meditech and/or PHIL.
  • Assists with obtaining physician signatures for telephone and verbal orders.
      • Contacts necessary consultants requested by the physician
  1. Learning and Development
      • Orients new personnel as assigned.
      • Attends a minimum of six unit meetings per year, assuming responsibility for understanding and acknowledging minutes of unit meetings missed. Attends all required inservices and completes annual competencies.
  • Willingly completes other tasks when assigned.
  1. Environment and Patient Safety
      • Maintains patient and work environment to maximize safe/clean working space.  Reports all hazards to the charge nurse.
      • Adheres to proper precautions, techniques and safety according to Infection Control policies.
  • Works with, cleans up and disposes of body fluids, blood and secretions appropriately.
      • Able to respond to situations that may require de-escalation or self defense tactics.
      • Able to respond to situations that may require de-escalation or self-defense tactics.
  1. Supplies
      • Demonstrates stewardship of equipment and supplies by using them appropriately, including use of PAR Excellence system.
      • Maintains adequate supply of items in patient rooms and unit.


  • Performs other duties as assigned to meet the organization`s needs.


  • Hazardous Working Conditions
    • Tasks that involve exposure to blood, body fluids, tissues and other potentially infectious materials.  Category I tasks are not a condition of employment.
  • Environmental Surroundings
    • Located in an indoor area with frequent exposure to mild physical discomfort from dust, fumes, temperature and noise.
  • Physical Surroundings
    • Continuously stand/walk or lift/handle/carry material or equipment of moderate weight (20 to 50 lbs).



  • Education
    • Required:  High School graduate or equivalent
  • Experience
  • Required: 6 months experience in customer service or medical related field or behavioral sciences
  • Preferred: Experience with mental health patient population
  • Skills
  • Required:  Experience working with computers to include data and numeric entry
  • Licensure/Certifications
  • Required: Basic LIfe Support (BLS) within 1 month of hire date, De-escalation training within 6 months of hire. 
  • Preferred:  Certified Nursing Assistant or EMT-basic
