CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services.  CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. Is responsible to assist in the implementation, provision and coordination of social services offered to patients in the medical center.  All the criteria-based duties and standards within this document will be performed according to the department and hospital wide policies, procedures, and guidelines.

Essential Key Job Responsibilities

  1. Coordinates and directs social services to ensure patient needs are met.
  2. Interacts professionally with patient/family and involves them in formation of Plan of Care
  3. Participates in psychosocial identification and planning for complex/high risk patients
  4. Assesses, counsels, refers and coordinates intervention relating to cases of abuse, neglect, exploitation of children, vulnerable adults, domestic violence or sexual assault.
  5. Identifies resources needed and works with the necessary agencies to assist patient/family with application, referrals, etc.
  6. Responsible for participating in multi-disciplinary discharge planning team meetings to assess and arrange discharge needs or alternate level of care.
  7. Completes brief assessments on complex situations and patients needing extensive social work intervention.


Required Education for Staff Job Levels

Graduate of an accredited university with a Bachelor’s Degree in Social Work.  Licensed as a social worker in ND ( LSW)  or in the process of obtaining license within 6 months. Until license is received will work as a social work designee in accordance with ND Board of Social Work Practice Requirements.

Required Minimum Experience

Minimum of 1-2 years of supervised social work working directly with individuals.

Required Minimum Knowledge, Skills and Abilities

Strong listening, oral and written communication skills are important. Ability to operate computer, maintain records and develop reports.

Ability to manage conflict effectively and develop positive interpersonal working relationships with others, both within the department and with staff in other departments. Must be able to relate well with patients, families, physicians and other healthcare professionals throughout the medical center.

PREFERRED Education and/or Experience

Prefer experience in a healthcare setting, specifically hospital
