Overview

Patients Medical Center (PMC) provides inpatient and outpatient medical and surgical services to residents of Pasadena, Deer Park, La Porte, Baytown, and Clear Lake. The facility includes 53 medical/surgical beds, eight ICU beds and three endoscopy rooms, and offers a range of primary and specialized services—wound care, general surgery, gastroenterology, occupational health, heart and vascular, women’s services, diagnostic imaging, outpatient rehab services, and sleep disorders.g for those who are poor and vulnerable, and innovating how and where healing can happen, both inside our hospitals and out in the community.

Responsibilities

PRN Position

As a case manager with CHI health, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management. The case manager will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs. The case manager must be knowledgeable in community resources and alternate care facilities.

Qualifications

Education and Licensure:

Bachelor's Degree - Nursing OR ADN and 5 years of clinical experience

Registered Nurse (RN) licensure

 

Minimum Experience:

Three (3) years of clinical nursing experience in the acute care or medical setting OR ADN and 5 years of clinical experience
