The nurse in the Patient Intake Center/Call Center is responsible for patient placement, hospitalist assignment, critical test result communication, MD to MD communication for patient hand-off, curbside consults, and making follow up phones calls to patients after discharge from designated nursing units. The nurse is responsible for facilitation of patient thru-put and overseeing potential and problematic obstacles. The Patient Intake Center/Call Center is a centralized location where all communication from stakeholders in the thru-put process is funneled through, this includes, but is not limited to, nursing, housekeeping, leadership, physicians, and registration/admissions. The Patient Intake Center/Call Center nurse must keep up to date information shared to others via Teletracking and Meditech, and other forms of communication. The nurse works collaboratively with physicians and assists with screening patients for appropriateness of admission, using established criteria
ESSENTIAL KEY JOB RESPONSIBILITIES
1. Uses patient information to accurately assign patient placement based on condition, admitting diagnosis, etc.
2. Accurately takes patient information related to patient condition and takes any verbal orders correctly
3. Answers calls from physician offices, hospitals and other areas and departments in a timely manner. Is able to multi-task effectively
4. Though the use of Teletracking, notifies appropriate areas of admissions in a timely manner.
5. Other duties as assigned by management
Required Education: Graduate from approved Nursing Program
Required Licensure and Certifications: RN licensure
Required Minimum Knowledge, Skills and Abilities: Competency with computers, fax, telephone and photo copying required. Familiarity with the Epic & Teletracking system preferred. Excellent organizational skills required. Demonstrated ability to interact professionally with physicians and physician office staff.
PREFERRED Qualifications: Experience in bed management and EMR systems
