As a Population Health Pharmacy Technician, you’ll play a key role in making a real difference in patient care! You’ll help guide and support patients throughout their healthcare journey, from start to finish. Working alongside doctors, nurses, and other healthcare team members, you’ll focus on improving patient outcomes and preventing unnecessary hospital visits. You’ll be the go-to for helping patients and families with their medications and education, advocating for them with the medical team, and connecting them with important resources in the community. When needed, you’ll escalate any medication or health concerns to a pharmacist, ensuring everyone gets the care they need while reducing unnecessary hospital stays and resource use. It’s all about making sure patients get the best care in the right place, at the right time!

Responsibilities

  • Completion of assigned patient care coordination activities, to include but not limited to telephonic patient outreach and support in accordance with established care guideline and protocols, hospital or clinic based patient care coordination visits.
  • Completion of assigned quality improvement activities, to include but not limited to submission of health plan requested documentation and forms, medical chart review, and clinical data abstraction. 
  • Serves as an advocate and liaison between patient/family, facility and physician. 
  • Coaches patient understanding of their follow up plan and connects patients to appropriate resources of care. Escalates issues beyond scope to the appropriate team member. Seeks guidance appropriately from pharmacist. 

Qualifications

Required Education and Experience

High School Diploma or GED

Preferred Education and Experience

Prefer two years of experience as a Pharmacy Technician in acute and/or ambulatory settings.

Required Licensure and Certifications

PHARMTECH:NE

Required Minimum Knowledge, Skills, Abilities, and Training

Proficient skills in reading, verbal and written communication. Strong reasoning skills in clinical decision-making. 

Planning and organizing, initiative and ingenuity, creativity, collaborative relationship with team members, problem solving, and ability to interpret data.

 Public relations and promotion of positive morale are important. Knowledge of EHR and minimal computer skills.
