Responsibilities

In accordance with the mission and philosophy of Dignity Health, the Palliative Care Coordinator is a professional caregiver who assumes responsibility and accountability for the planning implementing, coordinating and evaluating palliative care services. These responsibilities include developing and maintaining strong relationships with physicians, hospital staff to deliver palliative and comfort care to seriously ill and dying patients. This positions is accountable for those functions, which enhance the delivery of palliative care through activities as education, standards and program development, quality improvement activities, consultation and care management. This position is responsible for promoting high quality, cost effective care for palliative care patients. Incumbents in this position shall comply with all applicable laws and rules including the Dignity Health Standards of Conduct and Dignity Health policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Dignity Health Standards of Conduct, Dignity Health policy or procedure or applicable laws/regulations and when requested, assist Dignity Health personnel in investigating all allegations of violation. When called upon, the incumbent shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program.

Qualifications

Requirements:

  • Three years-clinical nursing experience required.

 

Licensure:

  • Current California license as Registered Nurse is required (RN)
  • Clinical Nurse Specialist (CNS) preferred.
  • Current training in Basic Life Support (BLS) for healthcare providers

Preferred:

  • MSN or CNS is preferred.
  • MSN or CNS is preferred
