Responsibilities

The Patient Flow Coordinator coordinates and facilitates the non-clinical activities of the patient care area through acting as a liaison with the nurse, physician, and ancillary staff enabling them to focus on quality care and patient satisfaction.

 

• Coordinates unit by greeting patients and visitors, triaging phone calls, maintaining current information on locator boards; communicates scheduled tests and procedures for patients.
• Processes patients’ charts upon admission and discharge.
• May assist with medication reconciliation upon admission.
• May process medication calendars and micromedix for discharge portfolios.
• Coordinates and maintains nursing unit supplies; monitors patient confidentiality; maintains a hazard-free work environment.
• As appropriate, responds to patient/visitor requests and complaints; escalates requests/complaints to the appropriate staff as necessary.
• Ensures correct patient type and accommodation codes on daily basis on all patients.
• Assists MSW with patient handoff to outside facilities upon discharge, including processing of information to receiving facility.
• Assists with patient care/bed placement assignments.
• Assists Unit Manager with clerical responsibilities.

Qualifications

We require:

  • High school diploma, or GED.
  • Two years customer service experience, preferably in a healthcare setting.
  • CPR certification required.

Preferred:

  • Working knowledge of medical terminology
  • Excellent customer service skills
  • An understanding of diagnostic tests, coding, procedures, and preparation.
  • Demonstrated time management, organizational and problem solving skills
  • Strong interpersonal skills; excellent verbal and written communication skills; strong computer skills
