Responsibilities

 

Job Duties


1.1 Provide personal assistance for functionally and cognitively impaired patients: toileting (every 2 hours), ambulation, showering, grooming and transfer of participants.
1.2 Assist with food service and providing nutritional intake support as requested.
1.3 Document treatments and observations as requested.
1.4 Provide door to door transportation assistance for multiple participants throughout Yolo County including transfer support on and off center vehicles as requested.
1.5 Ensure that vehicle is well maintained for maximum safety to operate by performing daily inspections. Monitor and report all maintenance and repair needs.
1.6 Provide accurate and timely documentation of all transportation activities. Report all incidents in a timely manner.
1.7 Follow all traffic rules including no cell phone use while driving, no speeding or abrupt driving maneuvers.
1.8 Monitor for significant changes in participant's physical, cognitive and functional status and report observations to rehabilitation or nursing staff.
1.9 Assist participants on and off vehicles and drive center vans.
1.10 Ensure that all Adult Day Health Center laundry is laundered according to policy.
1.11 Be trained and willing to provide door to door transportation assistance for multiple participants throughout Yolo County including transfer support on and off center vehicles as requested.
1.12 Apply provided training entitled “Person Centered Care in Practice”. Be able to articulate and apply model components.
1.13 Offer assistance to participants for all needs in a timely and respectful manner and identify opportunities to provide additional support.
1.14 Communicate unresolved work-related problems using chain of command, consistently demonstrate effective problem solving skills and handle confidential information appropriately.
1.15 Demonstrate friendliness and courtesy to participants and co-workers. Model a service attitude toward others, take ownership in resolving problems and take the initiative to improve situations.
1.16 Contribute positively to the program, activity and therapy aide team to ensure team cohesiveness.
1.17 Attend and participate in appropriate in-service and department meetings as requested.
1.18 Perform other duties as requested in a professional and timely manner.

Qualifications

Minimum

  • Nursing assistant skills

  • Effective and therapeutic individual and group activities

  • General knowledge of working with adults with disabilities cognitive impairment mental illness traumatic brain injury and developmental disabilities

  • High School diploma

  • Minimum of one-year experience working with elderly and disabled persons

  • BLSCPR; Valid California Class C Drivers License

  • Interact in a positive nature motivate and promote independence

  • Work cooperatively with participants volunteers staff and other agencies

  • Understand participants’ health problems

  • Perform adequately under stress when confronted with critical or unexpect

Preferred

  • Experience providing personal care assistance such as toileting and showering preferred

  • Bilingual Spanish language preferred
