At Parkview Health at Home, we strive to embody our mission of delivering hope and healing to those we serve.We are seeking a dedicated Nurse Support Coordinator to join our team. In this role, you will provide vital support to the Director of Clinical Services and our clinical staff to optimize patient care. You will act as a clinical intermediary and ensure seamless communication between healthcare providers, patients, and administrative departments.

 

Key Responsibilities:

  • Assist in the completion of daily activities to enhance the efficiency of clinical staff.
  • Obtain authorizations and manage benefits coverage for patient services.
  • Complete intake referrals with accuracy and timeliness, ensuring all necessary information is gathered.
  • Update physician orders and communicate orders or changes to the appropriate field staff.
  • Entry of ICD-9 codes and manage OASIS assessment data entry accurately.
  • Liaise with the corporate Revenue Cycle Department to address queries and resolve issues as required.
  • Audit clinical charts and maintain clinical documentation, such as discharge summaries, in accordance with company policies.
  • Communicate effectively with external service providers (e.g., HME, supply vendors).
  • Assist in training and onboarding new staff members, identifying clinical training needs.
  • Participate in case conferences for interdisciplinary care coordination.
  • Perform client visits as directed and offer clinical expertise to internal teams.

Qualifications

 

  • Current LPN license in good standing for the state serviced.
  • Minimum of two years' experience in a clinical or support role.
  • Strong clinical skills and familiarity with multiple software programs, particularly Windows-based applications.
  • Excellent organizational, interpersonal, and communication skills.
  • Proven ability to work collaboratively within a multidisciplinary team.
  • Strong attention to detail with effective time management capabilities.
  • Valid Driver’s license and auto liability insurance as per organizational policy.
