Overview

The Immanuel Heritage Center's Inpatient Services Program is an acute, short-term program for patients who are in need of a secure, therapeutic environment 24-hours a day. A multidisciplinary team composed of nurses, social workers, occupational and recreational therapists, and registered dietitians working under the direction of a psychiatrist coordinate each individual's plan of care. Additional medical needs are provided by physicians within CHI Health.

Responsibilities

Eligible for up to a $5,000 New Hire Sign-On Bonus and up to $10,000 Student Loan Forgiveness!!!

 

  • Conduct and document psychosocial assessment of patients 
  • Share responsibility in identifying appropriate decision makers if the patient is without capacity, educating all patient’s rights and encouraging participation in decision-making and care
  • Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs 
  • Identifies mental health needs of patients and provides crisis intervention and referral as indicated 
  • Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community.  Informs other members of the health care team about the factors that may affect the patient’s health and well-being
  • Serves as a resource to patient, family, and healthcare team regarding emotional and behavioral issues and refers to community services available to adolescents, adults, and elders
  • Remains current of Medicare and Medicaid laws and guidelines, Medicare conditions of participation, managed care contracts, and other legal and regulatory requirements as the relate to healthcare social work services. Assures patients are informed of rights and responsibilities as participants in these programs

 

 

Qualifications

-Master’s degree from a school of social work accredited by the Council of Social Work Education REQUIRED 

-Certification in Social Work in the State of NE REQUIRED
