Is expected to perform duties in alignment with the mission and policies of the organization. Performs duties in support of and in compliance with the performance improvement plan Joint Commission and other licensing accrediting and regulatory agencies. Through screening mechanisms identifies patients who are at high risk for need of social services intervention. Provides assessments to identify bio-psychosocial emotional substance use maturational cultural adjustment financial and/or legal problems then intervenes by implementing suitable interventions. Facilitates linkage/assistance that enables patients/families to access medical care to achieve optimum levels of health and fulfill goals for treatment. Functions may also include discharge planning.
