Hello humankindness Dignity Health Medical Group is the employed physician group of Dignity Health Arizona. Dignity Health Medical Group (DHMG) employs approximately 400 providers and 1200 support staff that cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 subspecialty services. The physicians provide clinical services in their areas of specialty and many serve in pivotal academic research and leadership roles.DHMG is also heavily involved in preparing tomorrows healthcare providers. DHMG has 84 medical school students and approximately 200 residents and fellows throughout the 25 academic programs. Clinical services are complemented with translational and bench research to augment medical education for residents and students. The Nurse Navigator is assigned to a population of patients and provides individualized and comprehensive care to reduce barriers, coordinate diagnostic and treatment efforts and ensure prompt access to care. Applying the nursing process, the Nurse Navigator performs clinical assessments; education on disease process, plan of care and treatment modalities; and symptom management to support patients through the complexity of their care continuum. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to effectively coordinate patient care, ensure continuity of care and create a comfortable and exceptional patient experience. The Nurse Navigator initiates referrals to appropriate interdisciplinary teams for physical, psycho-social and spiritual support through all stages of the continuum of care.
MINIMUM
ADN
3 years of experience as a RN in an inpatient or outpatient setting or 1 year RN with 3 years experience coordinating patient care, navigating the healthcare system, providing psychosocial support to patients, and healthcare administration.
Strong knowledge of medical terminology
AZ RN license
American Heart Association BLS
Computer skills, EMR documentation, Record/Verify system, patient education/interventions, symptom management.
