Dignity Health Medical Group (DHMG) employs approximately 400 providers and 1200 support staff that cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 subspecialty services. The physicians provide clinical services in their areas of specialty and many serve in pivotal academic research and leadership roles.DHMG is also heavily involved in preparing tomorrows healthcare providers. **MUST RESIDE IN ARIZONA
The Population Health and Care Quality RN provides managerial oversight of managed care incentives programs (e.g. quality/HEDIS/PCMH) to maximize incentive capture. Coordinate with medical directors regional directors and clinical pharmacist within incentive programs. Serve as a liaison to hospital case mgmt. to facilitate care coordination post- acute to the transitional care clinic. Identify gaps in care via data mining EHR and facilitate patient appointments and appropriate follow-up (mammo HTN/DM etc). PCMH oversight. Manage Monthly PCMH meetings. ACN liaison & contact for navigator to prevent attribution loss. Must possess significant knowledge of quality metrics: Ambulatory Acute Care PCMH ACN and payor population health incentives.
MINIMUM
Bachelor's Degree or equivalent combination of education and experience.
5 years experience working in an Ambulatory/Acute Care environment including PCMH, ACN, Payor Population
Health Incentives and with quality metrics
AZ RN license
PREFERRED
Experience MIPS Ambulatory Quality and Ambulatory Regulatory Standards
Must be detail oriented and able to focus on multiple hospital and system operations.
Effective communication skills with diverse groups, including physicians, managers and employees.
Experience working with Microsoft Project or other Project Planning software.
Knowledge of hospital operations and planning ability.
