The Patient Logistics Registered Nurse (non-bedside) coordinates the transfer center activities for Dignity Health facilities in collaboration with others including hospital administration, nursing leadership or their designee, medical staff, physicians and transportation vendors.
The Patient Logistics RN functions as the liaison between referring hospitals into and Dignity Health facilities to coordinate the transfer and admission of patients to hospitals.
The Patient Logistics RN bases facility designation on diagnoses, planned procedure and level of care requirements and acuity.
Also participates in all activities, meetings, calls, and education to facilitate transfers from all points of entry to the acute hospital setting.
Note: this shift is from 7:00AM to 7:30PM weekdays and an average of 4 weekend shifts per month.
