Overview

CHI Oakes Hospital, is a 20-bed critical access hospital that provides various inpatient and outpatient services to approximately 14,000 people in southeastern North Dakota. It is also a 24-Hour Emergency Level V Trauma Center. The hospital building was newly-constructed in 2007, replacing a 50-year old building, and in 2010, Oakes Community Clinic was opened within the hospital building. The hospital is part of a larger values-based organization, CommonSpirit Health.

Responsibilities

As our Case Manager Coordinator RN with CHI Oakes Hospital,  you’ll work with physicians, other registered nurses, varied departments, and community resources. This provides patients with the education and resources needed to build a care plan at discharge. More specifically you'll monitor and evaluate patient care practices in the acute, emergency department, and swing bed units.  

 

You’ll build trust in these interactions by showing compassion, using your critical thinking skills, and remembering that each patient requires a custom care plan tailored to their needs. You will support patients by identifying and removing patient care barriers that will regress their healing. Your talent for creative thinking is vital to your success in case management. It shines as you develop enhanced discharge plans yielding optimal outcomes for each patient. 

 

What you'll do...

  • Facilitate positive clinical and cost effective outcomes.
  • Coordinate discharge plans with discharge planning team. 
  • Monitors and coordinates swing bed referrals.
  • Assists with developing complete care plans. 
  • Acts as patient advocate and assists with patient's needs.
  • Serves as a resource for the medical staff and nursing.
  • Works well clinically and interpersonally with the Medical Providers to support care coordination.
  • accountable to coordinate a patient's care across a continuum of care.
  • Additional duties as described in the formal job description. 

Qualifications

  • Graduate of an accredited Nursing Program. 
  • Bachelor of Nursing (BSN).
  • Active North Dakota Registered Nurse license.

 

Preferred

  • 3-5 years current clinical care experience in an acute care environment. 
  • Prior case managment/utilization review experience.
