Overview

Dignity Health – Arizona General Hospital located in southeast Mesa on Elliot Rd and Ellsworth Rd is a fully-equipped hospital open 24-hours, 7 days a week, 365 days a year. The facility features an emergency room, capacity for 50 inpatient beds, an Intensive Care Unit, 14 emergency room beds, four operating or procedure rooms, a full onsite laboratory and radiology suite complete with 64 slice CT scan, x-ray, ultrasound capabilities and MRI.

Responsibilities

Position Summary:
 
Under the guidance of the Department Director, the Care Coordination RN Manager is responsible for oversight of daily operations to ensure the efficient and compliant departmental execution of the companies case management functions. These duties include, but are not limited to: staff performance management, reviewing or gathering of data to promote performance improvement, participation in or facilitation of meetings or projects related to departmental functions.
 
Principal Duties and Accountabilities:
  • Manages areas including organizing and executing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reports needs within the Medical Center and Dignity Health, as well as government and regulatory reporting.
  • Implements Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the manager’s accountability
  • Consults and collaborates with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.
  • Oversees the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care.

Qualifications

Minimum:

  • Bachelor's degree in Nursing (e.g. BSN) or Masters in Social Work or equivalent education and experience 
  • Minimum of 3 years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or 3 years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination and discharge planning
  • Operational experience, implementation, and staff development in healthcare environment
  • Current unrestricted Arizona RN or LCSW license
  • National certification of any of the following: CCM (Certified Case Manager), ACM (Accredited Case Manager) required or within 2 years upon hire.

Qualified candidates will have the following knowledge and skills:

  • Apply clinical guidelines to ensure progression of care
  • Communicate effectively in writing and verbally
  • Must have analytical, critical thinking and problem-solving skills
  • Collaborate effectively with multiple stakeholders
  • Proficient in the use of computers and multiple software programs 
  • Actively work within electronic health record (Epic, Cerner, Meditech, and/or Paragon)
  • Understand how utilization management and care coordination programs integrate
  • Work as a team player and assist other members of the team where needed
  • Thrive in a busy, self-directed environment
  • Knowledge of CMS standards and requirements
  • Proficient in prioritizing work and delegating where indicated
  • Highly organized with excellent time management skills

We prefer candidates with:

  • Master's degree in Nursing, Health Care Administration or related clinical field preferred.
  • Experience with performance improvement activities
