Responsibilities

Position Summary:

 

Under the supervision of the the Transitional Care Center (TCC) operational and clinical directors, the RN Navigator serves as a coordinator of clinical care for patients requiring assistance and support in accessing and managing their health care needs. The RN Navigator connects with the patient and caregiver at regular intervals, driven by patient acuity, to assess for overall wellness, symptom burden, identify and resource for barriers to care, provide emotional support and patient education. The TCC RN collaborates with the supervising provider and serves as a liaison between providers/care settings to ensure the delivery of competent, compassionate, and age appropriate patient care to prevent unnecessary ED visits, acute admissions, or other untoward outcomes.#RN@DH

Qualifications

Minimum:

 

  • At least 1 year in a health care setting or equivalent education and/or experience in a customer service role with demonstrated verbal and written communication skills and the ability to connect with diverse communities.
  • RN CA

Preferred:

  • At least 2 years in a health care setting with accountability for working with patients in nursing and/or case management/care coordination 
  • Ability to act with empathy and communicate and follow up with the patient/family for continuity of care
  • Ability to work collaboratively with all care professionals including physicians, nursing, and community based organizations and post-acute care facilities
  • Candidates should have excellent technology skills and be able to effectively work in various software application programs.
