Responsibilities

Job Summary 


The Pharmacy Technician Medication History is responsible for the collection and documentation of a patient’s allergies and a current, accurate medication history. The Medication History Tech obtains medication information through patient interviews, review of prescription claim and Controlled Substance Monitoring Databases, applies pharmaceutical knowledge to the information retrieved, and follows up with prescribers and pharmacies to create an accurate medication history. As a bedside clinician, the Medication History Tech assists nurses, pharmacists, and physicians with medication needs in the ED and other duties within the scope of practice, such as obtaining emergent medications in addition to medication history responsibilities.

 

Essential Key Job Responsibilities

  • Collects and documents a patient’s allergies and a current, accurate medication history
  • During medication history collection, identifies prescription, non-prescription (OTC), herbals, implantable devices/pumps, and any illegal substances
  • Clarifies incomplete or inaccurate information with the dispensing pharmacy, the provider who wrote the prescription utilizing Controlled Substance Monitoring Database and Drug Claim databases (SureScripts, DrFirst, etc.)
  • Enters validated medication history information into the patient’s electronic medical record/patient chart in accordance with established procedures
  • Meets or exceeds Memorial’s quality control standards for accuracy and productivity based on completed medication histories.
  • Consults with clinical pharmacists on any unresolved issues or those requiring a pharmacist’s clinical judgment
  • Utilizes software to Identify and follow up on any missed medication histories or patients with follow-up needs
  • Assists nurses, pharmacists and physicians with medication needs in the ED and other duties within the scope of their practice, such as obtaining emergent medications
  • Performs all duties of pharmacy technician
  • Other duties as assigned by management

 

Qualifications

Required Education/Experience:

 

  • High School Diploma or GED equivalent
  • 3 years of Pharmacy technician experience in any setting (retail pharmacy experience preferred)
  • Successful completion of medication history and patient interview training program
  • Graduate of ASHP accredited Pharmacy Technician Program preferred or an Associate Degree or Equivalent Experience preferred

 

Licensure/Certifications:

 

Registered Pharmacy Tech with the Washington State Department of Health.
