Responsibilities

JOB SUMMARY / PURPOSE 

The Care Transformation Education and Practice Program Manager will collaborate with leaders and frontline staff across the System to advance and transform care delivery using innovative digital solutions to include virtually integrated care (VIC). This position will advance professional nursing practice through leadership and staff mentorship to promote a learning culture and spirit of inquiry. This position will also engage the interprofessional team in evidence-based educational strategies that promote successful digital solution integration. 

ESSENTIAL KEY JOB RESPONSIBILITIES 

Leadership Responsibilities

  • Acts as role model, clinical nursing expert, change agent, mentor and educational leader for clinical staff across the System. 
  • Uses expert communication skills to mentor and coach staff and promote a culture of excellence (Magnet) across the System. 
  • Leads as a nursing education expert demonstrating progressive thought processes in the development and implementation of clinical education programs across the System.
  • Provides clinical expertise and leadership to quality improvement initiatives utilizing digital solutions across the System. 
  • Facilitates change management strategies during the implementation of digital solution technologies across the System.
  • Fosters a spirit of inquiry in team members to promote the utilization of evidence-based practice and research. 
  • Promotes and engages the voice of the bedside through shared governance/decision-making and other activities across the System. 
  • Collaborates to address process improvement strategies.
  • Promotes and facilitates national branding related to VIC and digital solutions.

Education Responsibilities: 

  • Collaborates with local department leaders and bedside team members to promote initial and ongoing education strategies.
  • Promotes teamwork and collaboration skills through the use of evidence-based strategies (e.g. TeamSTEPPS(R), Safety First) across the System.
  • Engages interprofessional teams in all levels of simulation (low to high) from the pre-implementation phase through sustainment. Works with the local education department to ensure sustainment of simulation education strategies to assess competency. Simulation strategies must utilize clear objectives with measurable outcomes that are measured post simulation across the System.
  • Effectively utilizes the Plan, Do, Study, Act process with leaders and team members to address ongoing educational needs to each of the Regions implementing VIC and digital solutions across the System. Promotes the deployment of the MissCare Survey in all phases and integrates the results in conversations with leaders and bedside team members.
  • Contributes to the development and maintenance of educational tools and resources, integrating adult learning theories.

Qualifications

MINIMUM QUALIFICATIONS 

Required Education and 

Experience

Master’s degree in Nursing required 

Minimum 5 years clinical expertise experience with minimum 5 years’ experience in nursing education. 

Minimum of 5 years’ experience facilitating simulation strategies with multiple hospitals or within a System required. 

Previous experience serving as a mentor/preceptor required. 

Experience leading teams in multi-level organizations (i.e. small Systems) required.

Experience facilitating TeamSTEPPS/Safety First preferred. 

Previous experience in leadership and operations preferred. 

Previous experience in virtual care models or virtual nursing preferred.

Required Licensure and 

Certifications

Compact RN license 

ANCC Magnet approved certification required.

Required Minimum Knowledge, Skills, Abilities and Training

Previous experience serving as a mentor/preceptor required. 

Experience leading teams in multi-level organizations (i.e. small Systems) required.
