Responsibilities

Coordinates, supervises and implements patient care based on competence, professional expertise, knowledge and evidence-based practice. Practice autonomously, consistent with professional standards, Nurse Practice Act and hospital job description. Input, extract, and evaluate data for meaningful use and patient centered medical home. Responsible for direct patient care, to include rooming, obtaining vitals, patient assessment & education, and discharge planning.

 

  • Patient flow: Able to effectively coordinate physician time, patient schedule, and clinical duties of care team.
  • Demonstrate initiative and flexibility with day to day activities.
  • Demonstrates behavior consistent in promoting excellent patient satisfaction.
  • Able to effectively address and resolve patient/customer service concerns; adept at service recovery.
  • Identify yourself and your role to the patient and describe pending interaction with patient.
  • Provide status updates to patients/providers/team/management as needed.
  • Responsive to patient/provider/team/management requests within the department/SJMG/SJHMC guidelines.
  • Displays understanding and compliance with organizational standards surrounding assessment/reassessment, biophysical & psychosocial barriers.
  • Responsible for referral/authorization management within the assigned care team.
  • Procedure/Treatments: Understands and can demonstrate to others the use of all medical equipment, which could include catheter insertion, Botox/Baclofen, procedure prep, surgical & procedure set-up, point-of-care testing, spirometry, NST. Accurately assesses patient and completes all documentation necessary for patient preparation for procedure/surgery. Follows appropriate consent procedure.
  • Develops and promotes appropriate education regarding healthcare to patient/family. Provide patient education.
  • Collaboration with multi-disciplinary care team, including but not limited to: Inpatient services, insurance plans, accountable care organizations, social services, internal and external providers, and any other entities involved in the continuum of care.
  • Consistently adheres to medication safety and reconciliation processes and guidelines.

 

*Clinic position M-F 8-5. No nights weekend or holidays.

**Heavy in medication management with Botox and Baclofen.

Qualifications

MINIMUM

Bachelors Of Nursing

Completion of RN program

Greater than two years of RN experience

Registered Nurse:  AZ (RN:AZ)

American Heart Association CPR (BLS-CPR)

 

PREFERRED

BSN

Greater than two years of outpatient/ambulatory facility experience
