Job Summary and Responsibilities

As our Speech Language Pathologist, you will help patients in the home setting receive speech language pathology therapy assessments and treatments so they can improve their communication, swallowing, and cognitive functions, enhancing their independence and quality of life.

 

Every day you will evaluate and treat patients, provide training to patients, family members, and caregivers, and actively participate in multidisciplinary team interactions for coordinated patient care.

 

To be successful in this role, you must possess a valid California SLP license, demonstrate comprehensive clinical judgment, and maintain accurate, complete, and timely patient records in accordance with regulatory guidelines and organizational standards.

  • Evaluate patients accurately and comprehensively within five days of the physician order, adhering to departmental policy and regulatory requirements.
  • Develop effective, patient-centered plans of care with appropriate short- and long-term goals, obtaining necessary approvals and making recommendations to the healthcare team.
  • Administer safe and direct patient care, utilizing established therapeutic procedures and techniques, and continually reassess and modify treatment plans appropriately.
  • Record evaluations, daily treatment notes, and discharge summaries, ensuring accurate, complete, and timely patient records.
  • Communicate effectively with families, physicians, and other health team members regarding goals, progress, and rehabilitative potential, and explain treatment objectives to patients.
  • Terminate treatment appropriately when goals are met or progress plateaus, planning for discharge in adequate time for referrals and making appropriate recommendations.

Job Requirements

Required

  • High School Graduate or High School GED upon hire
  • Active Speech Language Pathologist (SLP) license in the State of California upon hire
  • Basic Life Support – Cardiopulmonary Resuscitation (BLS-CPR) upon hire
  • Driver's License: California upon hire
  • Auto Insurance upon hire

Preferred

  • Two (2) Years Of Professional Experience as a speech therapist, working with adult and child language and speech disorders and swallowing
