  • Act as a resource to all clients, providers and the community.
  • Attend community meetings and health fairs to understand community issues and build relationships with community members.
  • Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Refer community members to needed health services.
  • Provides one to one coaching to those individuals desiring this service.
  • Teach classes or otherwise disseminate health information to groups, targeted families or individuals in a manner consistent with cultural norms

Qualifications

Education and Experience Required:

  • High School diploma/GED
  • Strong customer service orientation and the ability to handle highly confidential information essential.
  • Ability to work independently, set priorities and manage work within timeframes, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude.
  • Already persuing or completed Community Health Worker (CHW) training through ASU. 
  • AZ DL 

 

Preferred:

  • Bachelors Degree
