Responsibilities

At CHI St Vincent Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. We are looking for a compassionate and skilled RN Case Manager to join our team and help embody this mission. As an RN Case Manager you will deliver direct patient care based on approved treatment plans while adhering to clinical protocols and guidelines. 

 

  • Care for patients in their own homes!
  • Have the autonomy to create your own flexible schedule!
  • Care for patients around where you live!
  • We utilize Homecare Homebase electronic medical record which will save you time documenting!

 

Responsibilities:

 

  • Provides care and education to patients and their caregivers
  • Participates in the coordination of services with other disciplines and community resources as needed
  • Evaluates and revises care plans/assessments based on changes in patients and/or their environments
  • Participates in regular case conferences and in-service or special case orientations as needed and participates in on-call rotation.

Qualifications

  • Graduate of an accredited school of nursing
  • Currently licensed to practice in Arkansas
  • One year of RN nursing experience required
