Responsibilities

As a Community Health Patient Navigator, you will play a crucial role in the Emergency Department Syphilis/HIV/HCV Screening Program (EDSP), supporting routine opt-out testing and treatment for syphilis, HIV, and hepatitis C virus (HCV). Working under the guidance of the Program Manager, you will provide patient education, outreach, and linkage to care, ensuring that patients receive the support they need.

In this grant-funded position, you will collaborate with emergency department staff, local healthcare providers, and community organizations to coordinate referrals, advocate for routine STI testing, and establish effective follow-up systems. Your work will help normalize STI screening and improve health outcomes for diverse and underserved populations.

 

Key Responsibilities

  • Educate and coach patients on STI prevention, testing, and treatment options.
  • Facilitate patient referrals and linkages to care for syphilis, HIV, and HCV treatment.
  • Collaborate with ED staff and community health organizations to streamline patient support services.
  • Promote routine opt-out testing and advocate for patient-centered care.
  • Assist with program outreach efforts to engage high-risk and underserved populations.

Qualifications

  • High school diploma or GED required. Bachelor’s degree in Public Health, Health Sciences, or a related field preferred.
  • Two years of experience in community health education or healthcare, working with diverse and underserved populations preferred.
  • Ability to establish rapport with diverse groups, including the LGBTQ community.
  • Bilingual (Spanish) skills preferred.
