Responsibilities

About the Position:

In keeping with the Marian Medical Center health care philosophy, the Social Services Coordinator supports, communicates, guides and promotes the provision of quality patient care. Actively participates as a team member in the functions of the Extended Care Unit. The Social Service Coordinator supervises the Social Service Assistant and is responsible for the leadership of the Social Service Department Advanced training in the social sciences field.

 

Qualifications

Selection Criteria:

  • Advanced training in the social sciences field.
  • At least one (1) year experience in long-term care preferred.
  • Knowledge of QI/UR and social services requirements of OBRA, Title 22, and JCAHO.
  • Ability to communicate appropriately and work effectively with the residents, residents family, staff members and other community agencies.
  • The capability of making clear, concise and pertinent reports both verbally and in writing.
  • The willingness to use consultation and in-service training for growth and development.
  • Knowledge or ability to acquire knowledge of resources and services in the community available to residents and necessary for discharge planning.
  • Ability to evaluate and modify work environment/plan.
