Responsibilities

The TBI Navigator is an integral part of the rehabilitation team and Mercy Outpatient Rehabilitation Center. Navigator supports the participant by reducing fragmentation of care, improves compliance and access to care, supports efforts to reduce or remove treatment barriers, and assists participants in attaining their individualized service plan goals. The Navigator will help the participant direct their path through the continuum of care with the goal of improved care coordination amongst providers and reduce hospitalization, readmissions and ER visits. This position will involve telephonic management and direct participant contact along with data management and tracking of participant’s progress. Travel required.

 

Skills:

  • Basic understanding of ALOS, readmission rates, and gaps in care
  • Must be able to communicate clearly and concisely with all levels of individuals, sometimes in stressful situations
  • Must be flexible and able to adapt to changing patient and organizational priorities
  • Ability to manage conflict, stress and multiple simultaneous work demands in an effective professional manner
  • Demonstrates respect, concern, and empathy for the spiritual and emotional needs of patients

Qualifications

Required:

  • High school diploma or equivalent
  • Demonstrated experience in Microsoft Office, typing, and computer data entry

Preferred:

  • Bachelor’s degree
  • At least two years as Medical Office Referral Coordinator, Medical Assistant, or Health Plan Managed Care Organization and Utilization Technician experience preferred
