Responsibilities

Community Health Navigator - Community & School Health Initiative

 

Northridge Hospital Medical Center is seeking a passionate and dedicated Community Health Navigator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role for outreach and engagement to provide direct referrals to services for people who were the most affected by the pandemic. The goal is to further reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education.

 

  • Proactively reach out to community members and their families to provide information and support.
  • Conduct initial assessments to understand individual needs and connect them to community resources.
  • Educate community members on COVID-19 and other health-related issues through tabling events, presentations, and workshops
  • Build rapport and establish trusting relationships with community members and their families.
  • Advocate for community members' needs and rights, ensuring they receive timely and appropriate care.
  • Provide emotional support and resources to community members and their families.
  • Connect community members with relevant resources, such as financial assistance programs, transportation services, health, mental health and social services.
  • Oversee data collections, monitor data reports, and troubleshoot data issues
  • Monitor data reports, identifying trends, anomalies, and potential issues using the public health Outreach Tracker/System Navigator Tracker. 
  • Prepare regular data summaries and reports on outreach activities and the community
    SystemNavigation Referrals for internal and external stakeholders.
  • Other duties as assigned.

Qualifications

  • Bachelor's Degree in Public Health, or related field.
  • Minimum of one year of data entry experience required.
  • 6 months of experience working with the public or with community groups required
  • Excellent analytical, problem-solving, and communication skills (both written and verbal) required
  • Excellent organizational skills required.
  • Bilingual English / Spanish 
  • CA Driver License
  • Proof of Insurance
  • Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
