The Senior Patient Logistics Coordinator (PLC) supports the nursing team in areas of facilitating outbound transfers, transportation needs, transfer
request call intake and overall transfer process.
Responsible for managing the coordination and pre-processing of all patient admissions and/or internal placement of patient admissions, transfers.
Collaborates with Patient Placement Coordinators, House Supervisors and Site Leadership across the Division to ensure timely access to care for incoming external patients and interfacility transfers across the Division.
The Sr. PLC functions as a facilitator of throughput by expediting appropriate placement of patients within Dignity Health facilities across the Division.
This position incumbent's effective communication and interpersonal skills assist with the process of efficiently and effectively coordinating movement of patients throughout the designated Dignity Health facilities. In addition to facilitating and managing internal facility throughput; the PLC functions as the liaison for inter-facility transfers for direct admissions
