Responsibilities

Job Summary:

 

The Cardiac Rehab Nurse is responsible for providing specialized nursing care to patients enrolled in cardiac rehabilitation programs. This role focuses on monitoring patients' cardiovascular health, providing education on lifestyle modifications, and assisting in the development of personalized rehabilitation plans. The nurse will work closely with a multidisciplinary team to ensure the safe and effective recovery of patients who have experienced heart disease, heart surgery, or other cardiac events.

 

Patient Assessment and Monitoring:

  • Conduct comprehensive cardiovascular assessments for patients entering the cardiac rehab program.
  • Monitor vital signs, EKGs, and overall patient condition during rehabilitation sessions.
  • Evaluate patient responses to exercise and provide immediate intervention if needed.
  • Ensure proper use of medical equipment, such as heart monitors and defibrillators, during exercise sessions.

Patient Education and Support:

  • Educate patients and their families about heart health, medication management, diet, exercise, and lifestyle changes to prevent further cardiac events.
  • Promote and reinforce adherence to prescribed rehabilitation plans.
  • Provide emotional support and counseling to patients adjusting to a cardiac diagnosis and recovery.

Care Coordination and Documentation:

  • Collaborate with cardiologists, exercise physiologists, dietitians, and other healthcare providers to create and implement individualized rehab plans.
  • Document patient progress, response to treatment, and any significant changes in the patient's condition.
  • Communicate effectively with healthcare team members to adjust the plan of care as needed.

Patient Motivation and Advocacy:

  • Encourage patients to remain motivated and engaged throughout their rehabilitation journey.
  • Advocate for the patient's needs and ensure they have the resources and support they require.

Qualifications

Minimum:

 

Education:

 

Registered Nurse (RN) with a current and valid license.

Bachelor of Science in Nursing (BSN) preferred, or a commitment to pursue BSN within a specified time frame.

 

Experience:  

  • Previous experience in cardiac nursing, cardiovascular care, or rehabilitation preferred.
  • Experience with cardiac rehabilitation programs is highly desirable.
  • BLS and Advanced Cardiac Life Support (ACLS)- American Heart Association certification required.

Skills and Abilities:

  • Strong understanding of cardiovascular conditions, treatment protocols, and rehabilitation strategies.
