Overview

The Nurse Support Coordinator I supports the location’s Director, Clinical Services and team members in the completion of daily activities to optimize the clinical staff’s time in caring for patients.

 

The Nurse Support Coordinator II additionally, facilitates the completion of documentation to comply with regulatory, operational and financial requirements.

 

The Nurse Support Coordinator supports the Director, Clinical Services and team members in the completion of daily activities to optimize the clinical staff’s time in caring for clients.

Responsibilities

    1.     2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information. Maintains confidentiality of patient information.
    3. Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services.  Notifies Director, Clinical Services (DCS) as needed to ensure necessary information and documentation is obtained.
    4. Completes the intake referrals according to established processes ensuring complete, accurate timely information. 
    5. Acts as a clinical intermediary for the field staff including but not limited to:
    1. Updates or changes in physician orders
    2. Takes laboratory results and communicates results appropriately
    3. Appropriately enters ICD-9 codes
    4. Ensures the processes for physician orders (485’s), recerts, OASIS assessment data entry, updates and changes are completed accurately and in a timely manner.
    5. Acts as a liaison with corporate Revenue Cycle Department to resolve questions and issues, provide necessary documentation, etc.
    6. Takes referrals and completes the intake according to established processes ensuring complete and accurate information.
    7. Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services.  Notifies Director as needed to ensure necessary information/documentation obtained.
    8. Audits charts as requested.
    9. Completes clinical documentation, (i.e. discharge summaries, etc.) according to company policy.
    10. Communicates with additional service providers, (i.e. HME, supplies vendor, etc.) as needed.
    11. Assists in training new personnel as requested by management.  Assists in identifying clinical training needs.
    12. Assists the Director in review of processes and paperwork as requested. Completes required branch and/or company reports as required.
    13. Acts as a resource for external customers, including but not limited to, physicians, referral sources and discharge planners/social workers.
    14. Facilitates the resolution of client clinical issues.
    15. Performs client visits as needed and directed by management
    16. Provides clinical expertise for internal customers.
    17. Supports coordination of care through case conferences and other activities for interdisciplinary care.

    4. Completes patient visits as needed and as direct by branch leadership. 
    5. Assist with Telehealth program as directed by Director, Operations or DCS. 
    6. Acts as a liaison with corporate Revenue Cycle Department to resolve questions and issues and provides necessary documentation, etc.
    7. Communicates with additional service providers, (i.e. HME, supplies vendor, etc.) as needed.
    8. Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services.  Notifies Director as needed to ensure necessary information/documentation obtained.
    9. Assists in training new personnel as requested by management.

    11. All other duties as assigned. 

    Nurse Support Coordinator II 

    1. Performs a wide range of functions critical to the success of the organization’s branch location including but not limited to:
    1. Customer and clinical support.
    2. Branch Intake support and scheduling.
    3. Data entry and medical records entry for patient Health information.
    4. Talent acquisition support such as on-boarding and orientation help.
    5. Assists with payroll processing, as directed by branch leadership.
    1. Serves as a primary point of contact and coordinator of activities for all external and internal customers, including patients, other health care providers and branch staff. 
    When requested by branch leadership, the Nurse Support Coordinator II will also act as the Office Support Coordinator. The Nurse Support Coordinator II must review and adhere to the job descriptions duties and job expectations detailed in the “Office Support Coordinator” job description. 

Qualifications

Current LPN license in good standing to operate in the state serviced, 2 years of related experience
