Responsibilities

Under the general direction of and upon physician referral, to provide and maintain quality patient care to a select group of patients by utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. Contribute to the safety and maintenance of current standards of cardiac rehabilitation practice, which include attention to appropriate patient selection, concurrent medical therapy, patient education, careful prescription of exercise intensity and electrocardiographic monitoring to ensure an optimal arena for lifestyle changes.

  • Competently performs and charts a nursing assessment on each patient, based on patient's past history, current physical finding and age group, to include adult and geriatric. Plans care of a group of patients based upon nursing assessment and pertinent medical information form the referring physician. Based upon the clinical response of the patient relative to the treatment plan, complete progress and discharge documentation accurately and in a timely manner, using professional terminology.
  • Monitors process, provides patient/family education with a variety of planned learning experiences that facilitate changes in knowledge, attitudes, and behaviors with the goal of prevention, maintenance and/or improved status. Documents in the patient record patient and family needs, ability and readiness to learn.
  • Adheres to professional and ethical standards of the nursing profession. Identifies normal and abnormal pathophysiology and recognizes abnormal physiological responses and cardiac rhythms. Identifies the patient's educational, psychosocial
    and physical needs in order to establish priorities for the patients in accordance with their age, disorder and ethnic background.
  • Develop a comprehensive plan of care with the patient and family to meet the changing needs using the assessment database of cardiac risk factors and nursing diagnosis/problems. Plan and/or coordinates education for inpatient and/or
    outpatient and family education.
  • Acts as a liaison between the attending physician and team members to insure the patient and support persons are receiving all team services offered by the Cardiac Rehabilitation department including exercise programs, education and wellness programs and treadmill testing services. Follows departmental protocols and physician orders in identifying and evaluation patient needs based on information received according to the prescribed medical/exercise regimen. Assists physicians with special procedures as instructed.

Qualifications

 Minimum:

  • Must possess and maintain a current Registered Nurse license with the state of California.
  • Must possess and maintain a current Basic Life Support (BLS) and ACLS Certification, we accept American Heart Association
  • Demonstrated clinical competency in a manner approved by the California Society for Cardiac Rehabilitation

Preferred:

  • Knowledge in cardiovascular clinical nursing 2.Knowledge of twelve-lead electrocardiogram and/or monitor and telemetry monitoring equipment.
