Overview

St. Luke's Health–Sugar Land Hospital offers the latest technology and treatments for residents of Fort Bend County and Southwest Houston. Our services range from heart care and neuroscience to robotic surgery. Our recently opened Family Birthing Center offers incomparable care and amenities in a spa-like environment to provide a VIP—Very Important Pregnancy—experience. Since 2008, our experienced doctors, nurses, and support staff have provided expert care to support our community.

Responsibilities

Develops strategies, coordinates day to day operations, and evaluates impact of the services provided on patients
referred to the program. Works with interprofessional team to facilitate overall programmatic growth.

 

1. Maintains the daily operations of the program.
2. Coordinates care throughout the program utilizing appropriate tools to ensure high quality, cost effective care is provided to all patients referred to the program.
3. Performs preliminary assessments and assigns service providers based on referral criteria.
4. Acts as a resource person and liaises with others on matters related to the program and the program’s services.
5. Partners with other clinical team members in assessing and supporting the needs of the patient/family who have been referred to the program.
6. Presents program data and outcomes to internal and external audiences.
7. Provides educational offerings to internal and external audiences on program services.

Qualifications

Required:

*Registered Nurse (RN)

*Three (3) years of experience in specific program area

 

Preferred:

*Bachelor's Degree preferred
