Bachelors Social Worker: Be a Pillar of Support in Patient Care!

Are you a proactive and empathetic Social Worker eager to make a profound difference in patients' lives? Our hospital is seeking a dedicated Bachelors Social Worker to join our integrated Care Coordination team. In this vital role, you'll provide essential social work screenings and interventions, ensuring our patients and their families receive holistic support and seamless care transitions.

 

In this rewarding role, you will:

  • Champion Social Well-being: Screen for social needs, identify resources, and connect patients to crucial financial, housing, and community support.
  • Navigate Complex Discharges: Play a key role in post-acute placement for challenging cases, collaborating with agencies and community partners.
  • Support Vulnerable Populations: Advocate for and assist patients facing complex family issues, mental health crises, substance abuse, and end-of-life concerns.
  • Empower Patients & Families: Educate and engage on health management, coordinate interdisciplinary care conferences, and ensure safe, developmentally appropriate discharge plans.
  • Collaborate for Impact: Work closely with MSW/LCSW staff, the Care Coordination team, and multidisciplinary healthcare providers.
  • Maintain Excellence: Ensure meticulous documentation and adherence to federal/state regulations and organizational standards.

Qualifications

Required

  • Bachelor’s degree from a school of social work accredited by the Council of Social Work Education
  • North Dakota:  Licensed Baccalaureate Social Worker (SWORKER:ND)

Preferred

  • At least one year of  healthcare experience
