Under the supervision of the Central Coast Chronic Disease Program Coordinator and hospital site supervisor, the Chronic Disease Management Program RN plans, organizes and arranges for services patients with Chronic Disease will require. Applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs. Manages web-based care management data-base and functions as liaison between the provider, community and community based patients with Chronic Disease. Primarily provides remote case management via electronic &/or tele-monitoring. May provide direct patient assessments. Utilizes nursing process and formulates interventions utilizing current clinical knowledge. Initiates and participates in patient, family and staff education. Actively markets the program and participates on pertinent Dignity Health and/or community committees. Demonstrates leadership capabilities. Major responsibilities are listed; however, other duties may be assigned.
