At AmeriMed Home Infusion Pharmacy, we strive to embody our mission of delivering hope and healing to those we serve. The Quality Coordinator is responsible for reviewing and approving all patient information provided by licensed professionals during essential care milestones—including Start of Care, Re-certification, Resumption of Care, Evaluation visits, and Discharge. This role promotes high-quality patient care by maintaining strong relationships with referral sources, patients, caregivers, and staff. Additionally, the Quality Coordinator assists with staff training and ensures compliance with organizational standards and regulatory requirements.

 

The Staff Pharmacist provides safe and appropriate pharmacy services to home infusion patients. This role assesses patient suitability for home infusion care, reviews and evaluates medication orders, labs, and other clinically relevant information, monitors patient progress, and makes recommendations as needed. The Pharmacist is also responsible for overseeing sterile compounding of medication and quality assurance.

 

Essential Key Job Responsibilities:

  • Assesses patients' suitability for home infusion care.
  • Monitors and evaluates patient-specific pharmacy care plans.
  • Receives and evaluates medication orders for appropriateness.
  • Ensures medication-specific laboratory tests are ordered, results received, reviewed, and consulted.
  • Prepares and dispenses medications.
  • Manages the use, maintenance, and troubleshooting of all pharmacy equipment.
  • Provides medication-related education to staff, nurses, physicians, patients, and caregivers.
  • Assists in the training and supervision of pharmacy technicians, externs, and delivery personnel.
  • Participates in the on-call rotation.

Qualifications

 

  • Required Education: Registered Pharmacist/State licensure in state(s) serviced.
  • Required Minimum Experience: One year of hospital pharmacy experience. Experience with and knowledge of IV admixture procedures.
  • Required Minimum Knowledge, Skills and Abilities: Excellent communication skills (oral and written). Flexible and able to work with a multidisciplinary team. Excellent time management skills and able to work independently. Ability to function well as part of a team.
