CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services. CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. CHI St. Alexius Health offers a comprehensive line of inpatient and outpatient medical services, including: a Level II Trauma Center, primary and specialty physician clinics, home health and hospice services, durable medical equipment services, a fitness and human performance center and ancillary services throughout western and central North Dakota.

STATEMENT OF PURPOSE: Works under the supervision of the Staff Physical Therapist or the Physical
Therapy Supervisor. Assist Therapists as instructed. Carries out duties including charting, transcription,
scheduling, ordering/stocking, filing, faxing, charges, answering phone and cleaning booths. Inform therapist of
pertinent paperwork. Assist with Restorative Care duties as needed. Follow policy and procedures and the
Mission and Philosophy of Community Memorial Hospital.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
● Make new charts containing doctor’s order, prescription order, documentation sheet and functional
assessment. Make up schedule sheets. Charts reason for patient canceling appointment.
● Transports out-patients and in-patients to and from PT department. Prepares patients for therapy.
Assists in carrying out PT program under the supervision of the therapist in areas of gait training,
exercise, modalities, and wound care.
● Monitors, orders and stocks supplies. Straightens department daily, cleans and puts equipment in
proper place after each session. Disinfects department plinths and mats one time weekly.
● Ensures all Medicare forms and minor consent forms are signed. Check current charts weekly for
missed progress notes or re-certifications. Checks inpatient and outpatient charts daily.
● Remind therapist of an initial evaluations that are not done within 2 days of evaluation. Have notes
signed by therapist and provider and in place in all patient charts. Make sure all patients charts are
complete when taken to Medical Records to be filed.
● Opens, sorts, reviews and distributes mail accordingly. Answers phone and takes accurate messages
and directs calls appropriately. Answers routine questions regarding departmental policies operation
and procedures.
● Charges completed daily and taken to business office. Classifies, sorts and files correspondence
records and the documentation accordingly. Checks appointment schedules and directs questions to
appropriate office. Schedules appointments and notifies necessary department of personnel.
● Monthly stats completed on computer and printed. Maintenance record keeping and filing system in
accordance with department systems. Records department activity level and compiles monthly
reports.
● Assists in getting information together for preparation of reports or statements. Checks restorative
care charting and weekly notes. Assists Restorative Care when needed, and fills in when available.
Assists Cardiac Rehab for activities when needed.
Education: Technician training program at the Physical Therapy Department at St. Alexius Medical
Center.
Licensure: Training certificate.
Experience: Three years as a Physical Therapy Aide or college degree in Physical Education, Recreation
Therapy, Athletic Training or related fields.
