We are looking for a dedicated and empathetic Registered Nurse (RN) to join our Home-Based Hospice team in Park Rapids, MN. In this vital role, you will provide holistic, patient-centered care, bringing comfort, dignity, and support to individuals and their families during a sensitive time.

Responsibilities

As a Home-Based Hospice RN, you will be the cornerstone of our patient care, utilizing your expertise to provide comprehensive, individualized nursing services in the comfort of our patients' homes. You'll work autonomously yet collaboratively, fostering strong relationships with patients, families, and interdisciplinary team members.Comprehensive Assessment & Planning: Conduct thorough patient assessments to establish a health status baseline, develop individualized plans of care, and adapt to changes in patient conditions.

  • Collaborative Care Coordination: Partner with the healthcare team to seamlessly develop, coordinate, and implement the patient's plan of care.
  • Independent & Delegated Interventions: Execute independent nursing interventions and implement care delegated, ordered, or prescribed by licensed health care providers.
  • Safe & Effective Care Delivery: Provide high-quality, safe, and effective nursing care, promoting a therapeutic environment for patients and their families.
  • Patient Advocacy: Champion the best interests of individual patients, ensuring their comfort, dignity, and personal wishes are respected.
  • Continuous Evaluation: Regularly evaluate patient responses to interventions and the overall effectiveness of the plan of care, making necessary adjustments.
  • Interdisciplinary Collaboration: Work in close coordination with other healthcare professionals to manage and implement care across various settings.
  • Health Promotion & Education: Provide essential health promotion, disease prevention, and case management, including designing and delivering teaching plans tailored to patient needs and evaluating their effectiveness.
  • Professional Development & Accountability: Maintain clinical practice competency, participate in quality improvement initiatives, and assume accountability for the quality of care delivered.

Qualifications

  • Graduation from an accredited professional nursing education program.
  • Current Registered Nurse (RN) license in Minnesota (RN:MN).
  • Current Basic Life Support - CPR (BLS-CPR) certification.
  • Valid Minnesota Driver's License (AUTO:MN).
  • Proof of Auto Insurance (AUTO-INS).
