Responsibilities

This job is responsible for planning, managing and evaluating activities relating to the Continuing Care Network and delegated oversight for the Rainier Health Network (RHN) Care coordination. Work involves establishment of good relationships with network partners and providers in order to maintain voluntary adoption of, and adherence to, the RHN guidelines. An incumbent will effectively monitor adherence to established standards and basic data analysis. This position will further the efforts toward population management and effective systems of care coordination. Work includes overseeing the work of a subordinate outpatient care managers and coordinators.


This job requires a high degree of professionalism and the ability to influence/persuade/negotiate in order to gain acceptance of recommendations, ensure compliance with standards/requirements and facilitate achievement of clinical quality improvement goals. An incumbent must be able to conceptualize and apply sound analytical methods in order to spearhead program development activities, understand complex organizational impacts and address/resolve matters of significance, and challenges/problems encountered are often unique and with little precedent for effective solutions.


Key contacts are generally with VMFH management, physician leadership, steering committees, cross-functional or cross-organizational teams, community representatives and other stakeholder that have varying/conflicting priorities and where diplomacy, sensitivity, conflict resolution and consensus-building skills are critical to sustained relationship-building.

Qualifications

Education/Work Experience Requirements:

  • Bachelor’s degree in a healthcare-related field, graduation from an accredited school of nursing and three years of leadership experience that would demonstrate attainment of the requisite job knowledge/abilities; OR
    Minimum of five years leadership experience in the discipline; OR Master’s degree in a healthcare related field.
