Responsibilities

We're seeking a dedicated and skilled Transfer Center RN to play a pivotal role in ensuring timely hospital bed access for our incoming patients across all CHI-FH hospitals. In this crucial position, you'll be the central point of contact for emergency departments, healthcare facilities, and community providers, orchestrating a seamless transfer process that prioritizes patient needs and efficient resource utilization.

 

What You'll Do (A Day in the Life):

  • Be the Bed Navigator: Act as the primary liaison for incoming patient bed requests, coordinating placements with an expert understanding of regional bed availability and patient acuity.
  • Optimize Patient Flow: Continuously monitor bed demand, predict admissions, transfers, and discharges, and proactively identify available beds to ensure patients get to the right place at the right time.
  • Collaborate for Success: Work closely with a diverse team including site-specific patient placement staff, nursing, care management, and physicians to facilitate smooth transfers within CHI-FH and, when needed, connect with external hospitals for bed placement.
  • Problem Solve with Purpose: Quickly assess clinical information to determine the best-fit CHI-FH hospital and unit for optimal care, making informed decisions that consider patient needs and hospital resources.
  • Be a Data Detective: Track key patient throughput metrics, contribute to developing regional length-of-stay initiatives, and use data to identify opportunities for process improvements.
  • Build Strong Relationships: Foster excellent customer service with patients, families, physicians, and nurses, ensuring continued positive partnerships and referrals.
  • Champion Best Practices: Provide training and guidance to site-specific teams on patient placement and transfer standards, promoting efficient and standardized practices across our region.
  • Proactively Communicate: Maintain ongoing communication with all stakeholders, including patients awaiting beds, to ensure everyone is informed and prepared.
  • Resolve Challenges: Facilitate the resolution of immediate bed allocation concerns, investigating and addressing barriers to timely patient placement.

Qualifications

Experience/Education:

  • Associates Degree. Equivalent related work experience may be substituted for the Bachelor’s degree
  • Five years of progressively responsible related work experience, that demonstrates the attainment of the requisite job knowledge, skills and abilities preferably in a multi-hospital system
  • Critical Care or Emergency Room nursing experience
  • Charge Nurse experience preferred

Licensure/Certification:

  • Registered Nurse: WSDOH
