Responsibilities

Multiple Sclerosis Navigator  Part Time without Benefits - 39 hours during 2 week period

 

The Nurse Navigator of the MS Achievement Center (MSAC) is responsible, with input from the Program Manager and Director, for the education, support, and guidance of overall health and wellness for a person living with multiple sclerosis and their loved ones through the MSAC's day wellness programs and other services. The Nurse Navigator will be responsible for building collaborative relationships with the various healthcare systems in the Greater Sacramento area, as well as with MS organizations across the country, to provide the optimal level of support and service to people living with multiple sclerosis and their loved ones.

 

 

  • Delivers comprehensive education on overall health and wellness, employing diverse modalities for both group and individual settings to empower members with actionable knowledge.
  • Facilitates seamless access to essential resources and support for MSAC members, addressing their holistic health and wellness needs, including assisting with the enrollment process for new members.
  • Establishes and maintains effective communication channels with healthcare providers to promote MSAC offerings and collaborates with Multiple Sclerosis organizations to connect the MS community with valuable resources and support
  • Demonstrates an in-depth understanding of Multiple Sclerosis and its common comorbidities, such as diabetes, hypertension, hyperlipidemia, and cancer, to provide informed guidance and support.
  • Guides members and their families through understanding diagnoses, treatment options, and available resources, while proactively addressing psychosocial needs, advocating for optimal care, ensuring continuity of care, and identifying and mitigating barriers to access. Identifies and assists members in accessing relevant community resources and services that align with their established goals and desired outcomes, enhancing their quality of life.
  • Contributes to the development and delivery of specific educational programs, including assisting the MSAC staff with nutrition education initiatives.
  • Applies strong critical thinking skills to complex situations and demonstrates the ability to work independently and autonomously in fulfilling responsibilities.

 

Qualifications

Minimum: 

 

  • Current CA RN licensure and AHA BLS
  • 3 - 5 years of full time RN experience in an acute care setting working with high-acuity patient populations
  • Experience in providing formal and informal patient education
  • Computer skills utilized in the health care environment and Google Workspace
  •  

 

Preferred: 

 

  • BSN 
  • 1 - 3 years experience in community care and chronic disease management preferably with neurological diagnosis
