Job Summary and Responsibilities

The Community Liaison acts as the liaison between the community and CHSB. Is responsible for developing relationships with community providers to inform them of the nature and purpose of services offered. Provides assessments for hospital/partial hospitalization patients and referral information as needed. Integrates the program with the local mental health constituencies thereby improving the continuum of patient care. This position requires the full understanding and active participation in fulfilling the Mission of the Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino’s strategic plan and the goals and direction of the quality improvement, process, and activities.  

Job Requirements

  • Minimum of two (2) years experience in the unit’s specialty settings.  Prior experience as a Community Liaison/marketing highly preferred.
  • Experience working with the unit’s specialty and community outreach activities. Prior experience as a Community Liaison/marketing highly preferred.  
  • Associate or Bachelor’s Degree is required.  General knowledge of infection control standards and practice.
  • Broad knowledge of nursing principles, procedures and practice; ventilator skills/training a plus.
  • General knowledge of infection control standards and practices; member and participant in professional nursing organizations.
  • Presentation and public speaking skills a plus.
  • Computer knowledge/experience preferred.
  • Current LVN certification
  • AHA BLS required.
