Responsibilities

Are you a highly organized, compassionate, and detail-oriented professional ready to make a real impact on patient care? We're seeking a Care Coordination Assistant to join our dedicated Clinical Care Coordination team.

 

This vital role supports our RN Care Coordinators and Social Workers by managing critical, time-sensitive functions that ensure seamless patient transitions and optimal outcomes. What You'll Do:

  • Facilitate Post-Acute Care: Manage timely referrals, communicate with agencies, and ensure follow-up appointments to prevent readmissions and mitigate denials.
  • Patient & Family Liaison: Directly communicate with patients and families, providing essential information and securing necessary documentation.
  • Interdisciplinary Collaboration: Work closely with healthcare teams, patients, and external stakeholders to progress care efficiently.
  • Ensure Compliance: Uphold strict confidentiality (HIPAA), deliver regulatory notifications, and identify process improvements.
  • Administrative Support: Manage transportation referrals, insurance requests, authorizations, and crucial communications for licensed staff.

Schedule: Monday - Friday 8am - 4:30pm

Qualifications

What You'll Bring:

  • Required Education/Experience:
    • Associate's Degree (Business/Health) OR High School Diploma/GED + current healthcare professional license/certification (MA/CNA/EMT).
    • Medical terminology expertise (certificate or demonstrated knowledge).
    • Intermediate to advanced computer skills (Microsoft Office/Google Workspace).
  • Required Skills:
    • Excellent customer service and communication (written/interpersonal).
    • Strong analytical, problem-solving, and detail orientation.
    • Ability to manage multiple tasks independently in a fast-paced setting.

Preferred Qualifications Welcome: 3+ years healthcare experience, Bachelor's degree, experience in demanding patient care environments, relevant certifications.
