Job Summary:
 

We are seeking a dedicated and experienced Home Care Supervisor to lead our large progressive Home Health Agency which serves a diverse client base throughout the Sacramento Service area. As a Supervisor, you will lead strong multidisciplinary clinical teams and play a vital role in ensuring the delivery of high-quality patient care. 

 

 

Responsibilities:

  • Supervise and motivate an interdisciplinary team of home care professionals.

  • Act as a clinical resource, screening referrals and determining patient acuity levels.

  • Monitor staff performance, conduct evaluations, and implement corrective actions as needed.

  • Ensure compliance with all state, federal, and accreditation requirements.

  • Collaborate with physicians, interdisciplinary teams, and community resources to optimize patient care.

  • Assist in the development and implementation of patient/family educational materials.

  • Stay abreast of healthcare trends and recommend changes to promote quality and competitiveness.

  • Act as the MHH/MH administrator on call as scheduled.

  • Assist in providing in-service training and orientation for agency staff.

  • Represent the agency at internal and external meetings.

  • Supervise all clinical services and meet department-specific metrics to promote quality care and control operational costs.

  • Assist the department leader and Operations team with achieving and maintaining compliance with quality improvement, risk management, and utilization review strategies.

Job Requirements

Minimum:

  • Minimum of two (2) years of management/leadership experience (within the last 5 years) in a home health agency, primary care clinic or health facility, sufficient background knowledge and expertise in clinical decision making for the patient population assigned.

  • Current CA RN licensure and AHA BLS

  • Weekend coverage 0830-1700
