Supplemental Day Hospice Supervisor - weekend coverage 0830-1700
The incumbent supervises Hospice and Home Health service lines that contribute collectively to the provision of high quality patient care, while monitoring and supporting desired outcomes.
Responsibilities include:
Mercy Hospice is a Medicare-certified Joint Commission accredited program. We are also a member of the National Hospice and Palliative Care Organization (NHPCO) California Hospice and Palliative Care Association (CHAPCA) California Association for Health Services at Home (CAHSAH) and the National Association for Home Care & Hospice (NAHC). We are a partner in the We Honor Veterans (WHV) program. Mercy Hospice also contracts with many major private insurance companies that provide a hospice benefit. Regardless of ability to pay Mercy Hospice is available to patients and families who are facing life-limiting illness.
Our Mercy Hospice team consists of physicians nurses social workers counselors (including dietary) chaplains home health aides and volunteers. The hospice team meets regularly to discuss current cases and coordinate care to ensure quality and continuity for the patient. The medical director meets with the team to review care plans giving direction and advice as necessary. Additional services are provided such as pharmacy physical therapy and occupational therapy when needed.
Minimum:
2 years of management experience within the last 5 years in Home Care agency, primary care clinic or health facility
Sufficient background knowledge and expertise in clinical decision making for the assigned patient population
Current CA RN licensure, active CA Driver’s License, auto insurance and AHA BLS.
Preferred:
Leadership experience specific to Home Health and Hospice
Bachelor’s or Master’s degree in health or business related field
