  • Care Coordination & Education: Facilitate communication between patients, providers, and other healthcare team members; coordinate referrals to specialists, diagnostic tests, and follow-up appointments, and empower patients through education on disease prevention, health promotion, and adherence to treatment plans.
  • Documentation & Electronic Health Records (EHR) Management: Maintain accurate, timely, and comprehensive documentation of all patient encounters, assessments, interventions, and care plans within the electronic health record system, adhering to all legal and ethical standards for privacy and data security.
