Job Summary and Responsibilities

Position Summary:

 

 

The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Job Requirements

Experience: 

  • Two (2) years LVN Experience within the last 5 years
  • Recent experience as an LVN in a Home Health or Hospice Agency- preferred.

Licensure:

  • Currently licensed as a LVN in California
  • Current California driver's license, possession of automobile insurance, & access to an operating motor vehicle.
  • Current CPR/BLS card approved through the American Heart Association.
