Job Summary and Responsibilities

Join our dynamic healthcare team as a Social Worker (LCSW) specializing in Care Coordination! We are seeking a compassionate and skilled professional to provide comprehensive support and advocacy for our diverse patient population. In this vital role, you will play a crucial part in navigating complex healthcare systems, ensuring seamless transitions, and connecting patients with the resources they need to thrive.

 

  • Assess patients' psychosocial needs, barriers to care, and social determinants of health through comprehensive evaluations.
  • Provide crisis intervention and support as needed, demonstrating sound judgment and ethical practice
  • Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit.
  • Possess a strong knowledge of contemporary behavioral and systems theories relevant to health care, enabling effective assessment and intervention strategies.
  • Provide empathetic support for victims of abuse, neglect or violence, ensuring their safety and connecting them with appropriate protective services and resources

Job Requirements

  • Master's Social Work Required
  • At least one year of clinical and/or medical social work experience required.
  • Bilingual preferred. 
  • CA Licensed Clinical Social Worker (LCSW) required.
  • Fire Safety Card within 90 days of hire is required.
