Job Summary and Responsibilities

The RN Care Manager, in Radiation Oncology, is a vital member of our healthcare team, responsible for providing comprehensive care management services to patient populations grappling with chronic illnesses and/or multiple co-morbidities. This role is dedicated to improving patient care and optimizing health outcomes by empowering patients and their caregivers to effectively manage their health conditions, address related psychosocial challenges, and navigate the healthcare system with greater efficacy. The RN Care Manager promotes enhanced functional health status, seamless care coordination, and the elimination of redundant services, all while ensuring cost-effectiveness.

 

Recognizing the unique and diverse needs of each patient, ranging across the health-to-illness continuum, the RN Care Manager's practice is dynamic and patient-centered. This role demands proficiency in multiple dimensions of nursing practice to effectively support patients with varying levels of complexity. By synergizing nursing competencies with individual patient characteristics, the RN Care Manager strives to achieve optimal patient outcomes and support patients in reaching their self-defined optimal level of wellness, including a peaceful death.

Job Requirements

Requires:
  • Current Washington state licensure as an RN
  • CPR certification is required every two years
  • Bachelors of Science of Nursing
  • Strong patient assessment skills, ability to work collaboratively in team setting, current working knowledge of electronic medical records.
Preferred/Desired Skills/Background:
  • Experience managing complex patients or patients with chronic disease in an ambulatory setting.
  • Certification in Specialty area such as:
  • American Academy of Ambulatory Nursing in Certification in Care Coordination and Transition Management (CCCTM).
  • Oncology Nursing Society Provider Card for Chemotherapy/Biotherapy: Fundamentals of Administration
