A Care Navigator is a key member of the FullWell Care Management team that will support the RN care manager(s) and help outreach to patients following ED visits, in-patient hospitalization, and stable disease management patients. This role will coordinate with the patient’s primary care provider for medication refills, post- hospitalization appointments, and any RN delegated duties, etc. This role requires a strong telephonic communicator who can speak to patients to encourage/schedule preventive screenings, coordinate with primary care practices to schedule visits, assist patients with their care needs, and partner with their assigned RN care manager. We are looking for someone with initiative who works well with our Care Management team to ensure excellent quality patient care with great outcomes.

MA or LPN (Required)
Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally
