***This position will be hybrid in-office/clinic and work from home.
As the Supervisor of Care Coordination, you will be responsible for the oversight of day to day departmental operations including , but not limited to: clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review.
This position will supervise the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care. The Supervisor may assign work tasks, realign tasks within the department and take action to resolve minor disputes and productivity issues. This position may be assigned other duties within scope of practice, including, but not limited to : scheduling, conducting audits, performance improvement activities, assistance in staff evaluations or assuming a case load based on departmental needs. The position's focus will be on optimizing staff productivity, collaboration and compliance. The Supervisor provides support and coaching to staff members as indicated.
This position will report to and collaborate with the Manager or Director regarding identified issues outside the Supervisor's scope of authority. The Supervisor guides department activities according to the organizational objectives, hospital policies, standards of practice and Federal and State Regulations.
Essential functions include:
- Directly oversees and adjusts, when necessary, staff scheduling to ensure appropriate personnel resources for departmental operations.
- Provides timely support, coaching, counseling or corrective intervention within scope of authority as indicated, maintaining documentation and engaging Human Resources and the department Manager/ Director when appropriate.
- Performs ongoing assessments and maintains supportive documentation of staff professionalism, productivity, compliance, communication and collaboration to assist with the development of annual performance evaluations.
- Performs ongoing assessment of staff educational, resource and equipment needs and collaborates with the department Manager/Director to secure needed resources.
- Guides Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the supervisor's accountability.
- Considers the population served by the medical center and area clinical integration programs and leads efforts to optimize care coordination across the care continuum. This coordination ensures a plan of care for patients in all stages of health needs.
Minimum Qualifications:
- 3+ years in a Care Coordination or Case Manager role.
- Associates of Nursing degree.
- Clear and current CA Registered Nurse (RN) license.
- Comprehensive knowledge of care management and discharge planning, Medicare and Medicaid admission and review requirements, and general commercial admission and review requirements.
- Working knowledge of Interqual criteria
- Effective communication skills
- Knowledge of Conditions of Participation and Joint Commission standards.
Preferred Qualifications:
- Accredited Case Manager, Case Management Nurse, and/or Certified Case Manager preferred.
- Bachelors of Nursing (BSN) degree preferred.
